About Char Dermatology

Bridging the gap between clinical expertise and documentation.

Bridging the gap between clinical expertise and documentation.

Char Dermatology grew from a gap I saw repeatedly in high-volume practice: providers delivering thoughtful, complex care while their documentation did not always reflect the work and decision-making behind it.

The issue was not clinical expertise. Documentation, E/M requirements, the Medical Decision-Making Table, and modifier use are rarely taught with the same rigor as patient care. Yet these details help communicate visit complexity and support accurate reimbursement.

I created Char Dermatology to bridge that gap. I work with dermatology providers and clinical teams to translate strong clinical judgment into clear, defensible documentation—and turn chart-level findings into practical habits they can use every day.

Practical education and individualized chart analysis

Service 01

Private Chart Consultation

Chart Review and Action Plan

Evaluate 20 de-identified notes to compare billed E/M levels and CPT codes with those supported by the documentation. Uncover documentation gaps, missed billing opportunities, and EMR-driven patterns that may contribute to underbilling. Translate findings into a personalized action plan during a two-hour strategy session, delivered virtually or in person.

Service 02

The Art of Documentation

Team Documentation Training — up to 5 attendees

Strengthen your team’s ability to select the appropriate E/M level using the Medical Decision-Making Table. Understand when Modifier 25 is appropriate and how to document relevant Social Determinants of Health. Apply these principles to dermatology cases that reflect the documentation decisions encountered in everyday practice.

Service 03

Signature Engagement

Team Training and Chart Review

Align one provider and up to four clinical staff members around shared documentation standards. Deliver The Art of Documentation training in a private, team-based setting. Review 20 de-identified notes to reveal provider-specific gaps and opportunities for greater billing accuracy. Reinforce implementation with a tailored action plan and 30-minute virtual follow-up one month later.

A practical process, built around your practice.

01

Consultation

Every engagement begins with a brief consultation to understand your documentation habits, practice structure, workflow, and goals.

02

Chart Review

For services that include chart analysis, providers submit de-identified notes and corresponding billing summaries in advance. Those encounters are reviewed for documentation patterns, missed opportunities, and areas where the complexity of care may not be fully reflected in the medical record.

03

Personalized Education

Findings are incorporated into a personalized education session with practical recommendations that can be applied directly to the existing workflow. Depending on the service selected, this may also include EMR optimization, documentation templates, smart phrases, and individualized guidance.

04

Follow-Up

The Signature Engagement includes a 30-minute virtual follow-up approximately one month later to review implementation, answers questions, and reinforce recommendations.
